Time of bug found: Today (25.04.2017 at 09:20).
The bot was trying to worldhop. I paused it and checked what was wrong, when i tried to press escape manually i couldn't do so. When i clicked on interface to open settings and select world hop, then the bot would start it's magic and did hop.

So tl:dr : the bot was trying to open the escape interface but couldn't. Possible fix (open that interface by visible interfaces on gamescreen after it fails to open >= 2 times)

I have actually ran into this a few times myself and your possible fix sounds like it would work well.

ive ran into this alot recently, mostly if you stop a bot or pause, hotkeys no longer work, unless you manually open settings and go to display mode and attempt to change from safe mode to direct x or opengl, then hotkeys start working again
 
located the issue (atleast for me) it is as though the alt key is being pressed repeatedly (even within windows 10, i opened start menu and it was flickering, also pressing L from home teleport would attempt to teleport with Alt+L shortcut), attempting to change display mode within RS seems to solve it for me though
 
more testing, only seems to happen if i Alt+Tab from rs client

for now, add a failsafe that times out after some failures like I did
